Outpatient Services

New Horizons Mental Health Services serves children and adults of all ages throughout Fairfield County who need mental health counseling and treatment for a variety of mental health diagnoses through the following programs.

Crisis Services

24/7 Behavioral Health Crisis Intervention is provided by New Horizons Mental Health Services clinicians via Mobile Crisis Teams. These services are available to the public in the community and through the county Crisis Line by calling 211. Walk-in and outreach appointments are available for urgent issues.

Individual and Group Counseling

All clinics provide individual psychotherapy and counseling services for common mental health issues, including depression, anxiety, panic attacks, family and marital problems, and work-related difficulties. Additionally, specialized services are available, including treatment for substance abuse, domestic violence, post-traumatic stress disorder, early childhood intervention, social skills problems, and communication training. Referrals from other agencies, physicians, or hospitals are welcomed but not required.

Psychiatric Care

Our medical/somatic department is staffed with full-time, board-certified psychiatrists (child and adult), psychiatric nurse practitioners, psychiatric nurses, and medical assistants. Services include:

  • Diagnostic assessment and treatment planning
  • Medication management and follow-up
  • Medication-assisted treatment (MAT)
  • Case management and community support services, including assisted outpatient treatment (AOT)

New Horizons Mental Health Services staff work closely with all local, regional, and state hospitals to provide timely follow-up care after discharge from their facilities.

Community Support Programs

Interdisciplinary treatment teams offer care of varying intensity for individuals with chronic mental health disorders. This longer-term program includes:

  • Care management and healthcare coordination
  • Individual and group counseling/psychotherapy
  • Self-care and skills training
  • Client education regarding behavioral health disorders
  • Crisis care management offered during evening and weekend hours
  • Hospital diversion and hospital aftercare services
  • Housing supports
