Our website contains e-mail links that allow you to easily send an e-mail inquiry or feedback to New Horizons. When you use the e-mail links, your e-mail address and the subject of your inquiry are provided to the intended recipient to respond to your specific request only. To further assure protection of your privacy, we do not respond to e-mails about treatment issues or appointments. Please contact your clinician or clinic for these type of question.

Please contact your clinician or any of our clinic sites for specific information about use and disclosure of personal health information and the Health Insurance Portability and Accountability Act (“HIPAA) as well as the most up to date New Horizons HIPAA policy.
a. Your Protected Health Information (PHI) may be used and disclosed to carry out treatment, payment and health care operations.
b. We will use your health information for treatment. Information relevant to your care obtained by staff members will be recorded in your record and used by our staff to assist in the planning and evaluation of your care.
c. We will use your health information for health care operations. Staff members, such as Quality Improvement, may use information in your record to assess the care and outcomes in your case and others like it. This information will be used in an effort to continually improve the quality and effectiveness of the health care and service we provide.
d. We will use your health information for payment. Claims will be sent to your insurer as well as other PHI necessary to process the claim.
e. You have the right to restrict the use and disclosure of your protected health information (PHI) or to revoke your consent. We will comply whenever possible and except to the extent that we have already taken action in reliance on your consent. You have the right to request that we communicate with you via alternative means or location.
f. You have the right to request, in writing, to amend any PHI in your record. If we cannot comply, we will include your request, statement and our refusal in your record.
g. Your PHI may be requested by the court system, or other regulatory agencies.
